Diclofeanc Potassium 50mg + Paracetamol 500mg Caplet

  • COMPOSITION

    Each film coated caplet contains :
    Diclofenac Potassium: BP 50mg
    Paracetamol: BP 500mg
    Excipients: q.s.

    It is a non-steroidal anti inflammatory drug. It has analgesic antipyretic and anti inflammatory actions.

  • Diclofenac Potassium

    Diclofenac is an inhibitor of cyclooxygenase. It also appears to reduce intracellular concentrations of free arachidonate in leukocytes. Its potency is greater than indomethacin, naproxen etc.
  • Pharmacokinetics

    After oral administration it is rapidly and completely absorbed. It shows substantial first pass effect and is highly protein bound. It is metabolised, excreted in urine and bile.

    Onset of action: Analgesic effect 1 hour, antirheumatic effect about 2 weeks.

    Duration of action: Up to 12hours

  • Paracetamol

    It acts on CNS to produce analgesia and anti-pyretic effect. It has negligible anti-inflammatory action in therapeutic doses. After administration it reaches to peak levels in plasma within 30 minutes to 2 hrs. It bounds to plasma protein approximately 20% to 50% and is metabolized largely in the liver. Glucoronide and sulfate conjugates are eliminated in the urine.

    Paracetamol has got no side effect of phenacetin and is the main metabolite. It is an analgesic, antipyretic equivalent to that of acetylsalicylic acid. Paracetamol has no effect on the prothrombin time and no significant changes in the gastric mucosa.

    When administered orally paracetamol gets rapidly absorbed from the gastrointestinal tract. Paracetamol is effective, analgesics and antipyretic.

  • Adverse Effects

    Nausea vomiting, epigastric discomfort, skin rash, peptic ulcer, fluid retention, oedema, impairment of hepatic function rarely.
  • Contraindications

    GI bleeding, active peptic ulcer, NSAID induced allergic asthma.
  • Special Precautions

    Impairment of hepatic, renal or cardiac function.

    Children, blood coagulation disorders.
    Recent proctitis.
    GI disorders.

    In Pregnancy: Contraindicated in the last trimester.

    In Lactations: Use with caution.

    In Elderly Patients: May be given in reduced dose.

  • Interactions

    • Lithium & Digoxin: Blood levels of lithium and digoxin increased leading to enhanced efficacy and possible toxicity.
    • Diuretics: Inhibits diuretics but efficacy of potassium sparing diuretics enhanced.
    • Methotrexate: Toxicity enhanced.
    • Salicylates: Efficacy of salicylates reduced.
    • Cyclosporine: Increases nephrotoxicity of both agents.
    • Hydantoins: Increases serum levels resulting in toxicity.
  • Indications

    Management of pain and primary dysmenorrhoea when prompt pain relief is desired.
  • Dosage

    Adults: One tab 2 to 3 times in a day.
